What it is used for
Treatment of pulmonary arterial hypertension in eligible patients. The exact indication, starting strength and later dose are determined by a PAH specialist according to age, body weight, liver function, symptoms and other medicines.
How it works
Endothelin-1 is a naturally occurring substance that strongly narrows blood vessels. Bosentan blocks endothelin A and B receptors, helping pulmonary blood vessels relax and reducing resistance against which the right side of the heart pumps.
How to take it
Take only according to the specialist's schedule, with or without food and at consistent times. Do not start, stop or change the dose independently. Required blood tests and pregnancy testing must be kept up to date throughout treatment.
Common side effects
Headache, flushing, nasal congestion, dizziness, ankle or leg swelling, palpitations, tiredness and reduced haemoglobin may occur.

Contact the specialist urgently for yellow skin or eyes, dark urine, persistent nausea or vomiting, unusual tiredness, right-upper abdominal pain, fainting, worsening breathlessness, rapid weight gain, marked swelling or signs of an allergic reaction.
Warnings and precautions
Bosentan can cause serious liver injury and embryo-fetal harm. Liver enzymes require regular monitoring and haemoglobin is checked periodically. It is contraindicated in pregnancy; patients who can become pregnant need reliable contraception and scheduled pregnancy tests. Hormonal contraception alone may be less reliable.
Interactions
Bosentan has major interactions. Cyclosporine and glyburide/glibenclamide are notable examples that may be contraindicated or avoided. It can interact with warfarin, sildenafil, rifampicin, azole antifungals, HIV medicines and hormonal contraceptives. Specialist medication review is essential.
Missed dose
Follow the patient leaflet or prescriber's instructions. If it is almost time for the next scheduled dose, do not take a double dose to compensate. Ask a pharmacist or prescriber if unsure.
Storage
